All participants can register online at communitybank.net/glow5k or by mail. If a participant
is under the age of 18, this form must be signed by a parent or legal guardian.

HEALING HEARTS CHILD ADVOCACY CENTER.
THE RACE WILL TAKE PLACE AT SNOWDEN PARK.
Registration packets can be picked up anytime between
March 4 at Community Bank Southaven.
